1. I acknowledge receipt of appointment to the rank of Lieutenant USN in the Navy for temporary service made by the President on 1 April 1946.

2. Notice of this temporary appointment was made by my reporting senior pursuant to the authority contained in AlNav No. 150-46. My BuPers file number is 177283.

3. This temporary appointment has not and will not be declined by me.

4. Two (2) certified copies of this letter are being furnished by me to the Disbursing Officer carrying my accounts.

5. I certify that my appointment to the rank of Lieutenant USN has not been previously withheld for any reason.
