1. I hereby acknowledge receipt of notice of my appointment to the rank of Ensign in the regular Navy, for temporary service made by the President on 15 June 1943.

2. Notice of this temporary appointment was made by my Commanding Officer pursuant to the authority contained in BuPers C/L 96-43.

3. This temporary appointment has not and will not be declined by me.

4. Two (2) certified copies of this letter are being furnished by me to the Disbursing Officer carrying my accounts in order that my pay may be adjusted accordingly.
