1. It is imperative that this r eport b e executed a t expiration
of leave by the Commanding Officer of the individual to
whom the leave is granted and forwa rded immediately.
Do not file a copy i n the Officer Service R ecord (Form
NAVPERS 3021). Care must be taken to i nsure that cor­
rect number of day s charged as l eave i s r eported.
2. Th e day of d eparture from s t a tion or duty counts as a
day of duty; th e day of r eturn as a day of l eave, except
when such r eturn i s made before 0900, in which case it
shall not be counted as a day of leave.
This r efers to leave
taken w h en not detached from ship or st ation.
3. ln cases of a uthorized delay counting as leave, this form
i s to be submitted in addition to one copy of each set of
basic orders w ith all endorsements thereon, as r equired
by BuPers Manual.
